3D Sports Physical Therapy accepts Venmo, Zelle, checks and cash. I am happy to provide Superbills. As an out-of-network Provider for all insurance plans, we require payment at time of service. If you would like Superbills to submit to your insurance for reimbursement of their out-of-network percentage, please know that you are responsible for knowing (1) if you’ve met your deductible, and (2) whether you need Pre-Approval prior to your evaluation visit.

Q: Do I need a doctor’s prescription to come to physical therapy?
A: In California with Direct Access you can see a Physical Therapist without a Prescription / Referral for 12 visits (or 45 days). If I note anything of concern I will refer you to a physician sooner.
Q: What should I wear to my first visit?
A: Something that exposes the area where you are having symptoms and can move in. If in the lower body, shorts or short tights. If upper, tank or other short-sleeved top, please. Bring shoes you do your activity(s) in. Ideally also a well-worn pair so I may look at the wear pattern.
Q: How long will it take for me to get better?
A: I will provide my estimate at the conclusion of my evaluation. We will work together to find a plan that works for you. Depending on many factors including compliance with home recommendations, this timeline can change and I will keep you updated on my impression so you may plan.
Q: I am not making enough to pay the prices. Is there anything I can do?
